At Care Connect Inc., we provide Behavioral Healthcare Services to Children, Adolescents & Adults who are experiencing significant disruptions in daily functioning and social & emotional well-being.

Our Services Include:

1) Psychiatric Evaluation:

At Careconnect, we provide a comprehensive psychiatric evaluation to individuals requiring the services. The assessment is usually the first stage of the treatment process, where we identify the root cause of the individual need and provide them with the help they need. Then, we use qualified professionals to provide the right interventions and make appropriate recommendations if necessary. Appointments can be scheduled by calling the office or visiting our website.

2) Medication Management:

We provide medication management by working with our psychiatrist, DNP, and CRNP-PMH to prescribe, re-evaluate and ensure that your psychotropic medications are effective and safe. Our professionals will evaluate each individual and make recommendations on their medications as necessary. Appointments can be scheduled by calling the office or visiting our website.

3) Psycho-social Assessment (including Individual, Group, and Family Therapy):

We provide a psycho-social assessment to individuals, families, and groups requiring services in the community. This is often the first stage of treatment in which our professionals identify your mental health needs and make appropriate recommendations on the following treatment path. Appointments can be scheduled by calling the office or visiting our website.

4) Individual, Group & Family Psychotherapy:

We provide therapy to individuals, families, and groups based on the outcome of their psycho-social assessments, which requires such services as a follow-up if required.

5) Psychiatric Rehabilitation Program for Adults, Adolescents, and Children:

  • Children & Adolescents PRP:
  • These are services designed to support and strengthen parenting - to improve the child’s behavior, academic performance, and problem-solving skills & teach social responsibilities. Services are provided after school and during the day when schools are closed for holidays. Here, we teach skills development & provide structured activities to assist the recovery process in four key areas of development:

    1. Life Skills:

    to promote healthy choices, improve & maintain individual personal care & develop assertiveness skills.

    2. Education Management Skills:

    to provide guidance, improve coping, conflict management & communication skills, and support in obtaining GEDs & degrees.

    3. Vocational Training Skills:

    to provide hands-on leadership and management skills development opportunities.

    4. Socialization Skills:

    to improve social skills and promote community integration through religious organizations, social & recreational opportunities, and referrals to community-based funded programs.

  • Adult PRP:
  • To promote independence & community living skills, facilitate individual’s rehabilitation & recovery utilizing these service tools:

    1. Behavior health home

    2. Intensive On-site

    3. Supported Employment

    4. Rehabilitation coordination.

    5. Individual and group sessions.
